Hotbar Randomly replaces abilities with Materia Melding

Occasionally, my hotbar will replace random actions with the Materia Melding action. This happens when my hotbar is unlocked and I change classes, teleport, use return, view a cutscene or enter a new zone. The problem is intermittent however the replaced action is ALWAYS replaced with the Materia Meld action.

It often replaces between 1 and 3 actions. It seems to only affect hotbar 2. Most often it replaces actions when I am using my Monk. The most commonly affected slots are Hotbar 2 slots 1-3. However slots 8,9,0 have also been modified at times.

This happens to me all the time. I'm a PS3 player using the Cross-Hotbar, and Materia Melding will randomly replace my L2+Left command. It usually occurs whenever I change classes, but I swear that sometimes it happens when I warp to a new location. I've had this problem since Beta Phase 3, but I could've sworn it was fixed in Phase 4 because I never had the issue, then.
