Game minimising at random points during battle

As the title states, the game minimises and goes to desktop sometimes during battle.

This has now happened a number of times.

Once in Satasha while fighting the final boss, Once during a random mob fight and once (just now) during a fate encounter.

(Rest assured I am not accidently pressing the windows key)

I also experience a skill reset during battle (generally in lower ping areas) the timer (or darkening of a skill) will finish, I will use a skill and halfway through the cooldown it will say "Not ready yet" and reset back to full cooldown status. You can imagine the trouble with this problem during dungeons when I'm either tanking or healing (Spellcasting comes up as "Interrupted" when there is nothing to interrupt and I can only assume it is the same reason I am experiencing skill reset.

I have had this occur a few times as well playing on full screen. Random minimization. I just assume the game thinks I am too good and wants to handicap me. I am not hitting the windows key either, as my keyboard is a safe distance away from me and my controller.
