Hotbar bug "Adv. Materia Melding" puts itself on the bar

After learning any new skill, pet or ability, the icon of "Advanced materia melding" puts itself on my hotbar (controller crossbar), getting rid of whatever I had on it before. Locking the bar does not help, have to put the former ability back on every time. Also happens to my boyfriend on his game.
The melding icon goes to the LT group, up button.

So they know the problem is there, they are just not saying anything about it to individuals (or at all).
