I now agree with everyone else that this can occur on any off-GCD ability. I last posted on this a little less than a month ago, and I've tried to pay really close attention to when it happens. I still cannot reliably reproduce it, but I've narrowed down how it can occur.
When weaving off-GCD abilities the general moveset usually goes:
1) use ability --> 2.5s GCD begins
2) use off-GCD ability --> halfway through 2.5s GCD
3) 2.5s GCD ends --> use ability
However, if you are spamming the shit out of your moves (which we are forced to do to get them to activate), the following bug can occur:
1) use ability ---> 2.5s GCD begins
2) use off-GCD ability BUT ability does not trigger until 2.5s GCD has almost ended due to variation in triggering delays
3) off-GCD buff has triggered, but in the middle of the trigger your next on-GCD ability goes off due to button spam, interrupting the off-GCD action and moving into your regular ability instead
If you use the off-GCD ability right as the 2.5s GCD comes off, and too quickly move into your next ability, the off-GCD ability will trigger but fail to apply. This uses the cooldown, but gives you no buff because the application of the buff seems to be interrupted by the use of the next ability (the ability to interrupt off-GCD buff applications would be the actual bug itself). This only tends to occur during fights due to ability/off-GCD button spam, and I believe can only occur if the off-GCD ability is used right as a regular ability becomes available. You must be spamming both of them to create the interruption, and get unlucky enough that the off-GCD activation was delayed until the 2.5s GCD has almost ended.
