Eventually this will be fixed when ilvl restrictions come in 2.1. But there really is no excuse to be wearing 45 and below gear to a hard mode fight. There is no good argument for not properly equipping yourself with basic gear that would be suitable for the fight. Level 49 accessories are cheap cost maybe 1-2k.
