Teamspeak and vent is good enough.
Biased as I am, I don't like the idea of voice chat due to my hard of hearing. It'd never work out for me.

Right now when I play, I'm stuck making group phone calls to have a suitable form of communication. Luckily I have unlimited minutes on my cellphone contract (love AT&T) with unlimited group calls. I play on ps3 and don't own a computer so using third party programs like Ventrillo and Teamspeak are out of the question. I'm damn glad that my ps3 Bluetooth works amazingly well with my iPhone, because I wouldn't be able to play and communicate without it. Having VOIP that works natively across PC and ps3 would be amazing. It would save me a lot of hassle.

every MMORPG that has in game voip is hardly ever (if ever) used though. And as such is usually regarded as a huge waste of development time and resources.
The problem is that usually those who are being disruptive are the only ones who need to be told what to do on a regular basis. Being able to mute members works both ways and can be used by those disruptive members to mute any "voices of reason" while they continue trolling the party.
Nevermind the fact that as is, we don't have any easy way to blacklist those people who spam in cities, so any means of muting an individual member would likely be more work than its worth, which would render any such system useless.
