Monster attack after selecting Return being KO'd

Area and Coordinates: Upper La Noscea > Iron Lake (18, 10): tunnel to Oakwood.

Monster Name: Smolenkos
In-Game Time: Cannot record in-game time
Steps:

1. Encountered a few Smolenkos in Upper La Noscea, inside the tunnel leading to Oakwood.
2. Hit "Quickstride" ability and try to run past 2 Smolenkos. (I knew it was not going to work, haha)
3. Got agro by both of them and died inside the tunnel near the exit.
4. Being KO'd, I selected Return option from Main Menu.
5. My character got to his feet and was killed one more time by a nearby Smolenkos.
6. My character appeared KO'd at Home Point (Camp Skull Valley)
7. Hit Return from Main Menu once again.
8. Got black screen ("Now loading...") for several minutes so I rebooted.
