I had all of these problems and more. After talking with my bank I found out that only playspan was trying to charge my card (and locked it). Global Collect never even attempts to charge the card (no activity on my account from them).

I finally gave up and bought an Ultimate Gamer Card like many other people. That got rejected. I contacted UltimatePay customer support and they pushed the transaction through for me (they asked for the card PIN and transaction ID, I only had the PIN because square's site doesn't give any useful information when stuff fails).

Just so everyone knows this had been reported long ago http://forum.square-enix.com/ffxiv/t...601-CE-Upgrade and it still hasn't been addressed, so don't expect them to fix it any time soon. This shit never worked correctly with 1.0 and it seems they still haven't learned their lesson. How can you fix a game and not fix your billing system????
