Disappearing/stuck party member

I haven't had this issue happen since I played during Phase 3, but there are some instances where I'm in a dungeon and a party member either disappears off my screen or is stuck in one location. When I open up my map, they're icon is way behind from the rest of the party, but I can see an area where monsters are gathered. I can target them on my party window, but I can't heal them. The only way I can heal them is to spam medica. Only way the issue gets fixed is if I die or the party members dies which I want to avoid as much as possible as healer. Anyone have any fixes or ideas that could be causing it? I'm playing on the original launch PS3 and yes I've checked my targeting filters.
