I don't know how people have missed these 2 MASSIVE posts by the Admin on this site but perhaps this will solve the mystery of how you got your accounts hacked?

Warning Concerning the Security of Your Account
Currently, we have confirmed that a third party is using account names and passwords, thought to be obtained from security breaches of other companys' online services, in attempts to gain unauthorized access to Square Enix accounts.

If you are using the same account name or password as your Square Enix account on other online services, there is a much greater chance that a security breach at any of the other online services could potentially lead to your Square Enix account being compromised.

Additionally, even if you are not using the same account name or password across multiple accounts, if you use repeated strings of character, phone numbers, birthdates, or other information that may be tied to your identity, this may make it possible for individuals to access your account simply by guessing its password.

Therefore, if you are using the same account name or password for your Square Enix account as for your other online accounts, or are using a password that might be easily guessed, we ask that you change your password immediately by going to the following page:

Also, please note that accounts that are suspected of having been compromised will have their access temporarily restricted. Once login has been restricted, an email notification will be sent to the primary email address registered to the Square Enix account. This email will contain instructions on how to remove the login restriction. Players will be asked to reset their passwords and log in again.

By Japanese and US law, businesses are required to announce any such security breaches, not necessarily within seconds of discovery, but at some point. Until SE does announce such a thing, I'll assume SE's only "fault" is a lack of tools to help GMs get rid of the spammers more quickly.

It's not the fault of SQEX if a lot of people in question use the same usernames and passwords on other websites and forums.
A lot of them have appalling security measures or run outdated software which is as leaky as a basket.
It's like writing your PIN code on your bank card, leaving your wallet somewhere and then blame your bank that the one that found it used your card to empty your bank account.

You can't imagine how many people actually use the same user and pass for every single thing.

never underestimate how dumb some people are with their passwords... like the same easy password from a game to a mail or paypa/bank account...

Forget laws. They're never applied unilaterally, so by anybody in a higher position are more like guidelines.

I'll take greater faith, that such a thing would be communicated simply as disclosure of personal failure (especially as affects others) is the honerable thing to do.

And SE permits your account login to use your primary email address. During beta I had to use a unique user ID, but now that I'm providing credit card information to my account again via Mog Station they allow me to use my primary email address to login. Great idea guys!

People aren't protecting themselves, but SE doesn't seem to be going out of their way to help.

Could you please Name these "Other Services" so i can check if i have one and do something about it?
