I've had a similar problem as well.

Character: Quintessa Nadaway
World: Hyperion
Job: Lvl 50 Bard
Progress: Defeated Garuda
Timeframe: Didn't notice exactly when it disappeared, but I took note of it missing on Oct. 12th.
