    The quest was gone after wiping on Garuda (HM) and exiting the duty. I spoke to the NPC and he just offered the quest from the very start.

    Bosses Defeated:
    D'horme Chimera
    Hydra
    Ifrit (HM)

    Was up to the stage of killing Garuda (HM) and the 'White Hot Ember' from Ifrit is gone.
