I think that people are misunderstanding how easy it is to get a Keylogger.

You don't have to go to a Gil-selling website. You don't have to go anywhere sketchy at all.

All you have to do is mouse over an ad. And this ad can be on a perfectly legitimate website - because those websites usually sell ad space to large providers who don't actually police what goes through that ad stream. If your computer isn't properly secured, all it takes is a single swipe of your mouse across an ad while looking for something else and the program embedded in the ad itself will download and install.

This is why the authenticator/security token is so important.

The other hugely important thing is that your game password, your email password, and the passwords you use for any fan sites are all different.

We, as players, can take steps and precautions in order to protect our accounts. That being said, not everyone who has lost their account is at fault or complicit in it happening. Bad things happen. It's in the interest of RMT to get access to your account, and they'll come up with new and creative ways of doing it, and doing it aggressively.

The amount of RMT spam coming from seemingly hacked/legitimate characters is incredibly disheartening.
