Not at all!
I've only experience the best of the best veteran MMO damage dealers! I love it when they:and the list goes on. (/sarcasm end)
- activate Internal Release + Raging Strikes even before I am in range for Shield Lob
- attack secondary target because, "tanks should not have a problem with threat, l2p"
- GOGOGOGOGOGOGO and disappear off the mini-map even as the healer starts to cast Protect
- stand in cleave zone and "lol tank do your job plz"
Yesterday I was on my Conjurer (just made WHM) and 10 out of 10 archers try to kite in the dungeons when they over-taunt. I feel sorry for them because none of them were anywhere close to half successful.
Regarding Provoke.
Provoke literally puts you on top and does a +1 to your enmity. Now there's nothing inherently wrong with that. What's bound to go wrong is the fact that all periodic effects occur based on fixed server times, not based on how long since they were put up. So you can have HealA, HealB, HealC all cast regen on different people at different times, but all three regens will tick at the exactly same time.
What happens then is that just as you got your +1 enmity, regen ticks and overtakes your enmity again.
WOW was better in this respect because of the 10%/30% aggro threshold, but FFXIV doesn't have this, so Provoke can fail sometimes if you aren't able to follow up with a real punch (Shield Lob/Tomahawk) immediately.
