Has anyone actually used a Phial of Fantasia yet?

I know some people have the veteran rewards already so I ask because I want to know exactly what it lets us change.

Is it literally just our character's race/gender/appearance that can be modified? Or does it let us choose a new birthday and deity as well?

I used mine earlier, I'm pretty sure you can change everything except name birthday and deity unfortunately. I was spamming through it cause it was when the 3102 error hit so had to try over and over to get it to work, but I remember the bottom 3 options were greyed out so you were unable to change them. Hope this helps a little bit, sorry I can't say for sure I only used mine to change a couple of options and never really thought about birthday etc.

Edit just to add - I think it's available on all characters when you create them so you could always create a new character just to check what is able to be changed before you use it on your main. Saves wasting it if you can't actually change what you need.

Can any tell me if the Phial of Fantasia is a 1 time use and then is gone and you can never use it again ( on that char ) or can you redeem it again for the same char later etc?.

You most definitely CAN change name day and diety, because I did so. You just can't change name or starting class/city (probably because it would cause issues for what quests you've already completed).

Ah you don't know how good it is to hear that! I know the deity is irrelevant to most people but stuff like that bugs me if I don't think it suits my character... Not to mention 1.0 dev stuff told us they will be playing a big role in the story later on so choice of deity might not be as minor as people assume.

Fantasia grants you a single opportunity to edit your character.
You will be allowed to change your race, gender, appearance, date of birth, and patron deity.
*Your starting class and name cannot be changed.

I can't seem to get this to work, I removed all my gear as it said to do. Logged out, now it says cancel to access subcommand to change my character...but nothing happens.

Use the item in game, remove equipped items, log out and before you log back in on your character select screen choose your character you want to change sub commands (ps3 is square button) and edit. Make sure you go through it all and don't exit the screen back to the log in screen or you will lose it you can go back to the race screen but any further and you cancel it out
