A multi-monitor span is done using Eyefinity (AMD) or Surroundview (NVIDIA). The OS essentially treats your multiple monitors as a single display, which then in turn allows you to run the game across all three. The easiest way to accomplish that is through borderless windowed or full screen mode.
Here is my set up:
The chatlog is on the edge of the left monitor and the main menu is on the edge of the right monitor. I try to keep the center screen as uncluttered as possible.
I usually have the full map up on the right monitor. I also play with a Logitech G13 game board and G600 mouse, so my hotbars are set up in clusters that make the most sense to me.

My personal opinion, but the default UI works the best. It really just needs a higher resolution or ability to scale. A lot of the UI's that people posted on here seem really clunky, but each to their own
I'm all about minimalism.

I think the UI needs to be allowed to be made smaller as my resolution right now is 1280x720 and it makes it really difficult to even have things on so I've had to disable a lot of things in order to make the UI less cluttered.
