Are you sure Berserk is available to MNK? Haven't got a high enough MRD to test myself, but at least according to xivdb.com Berserk is not a cross class skill.
nope you cant use berserk but blood for blood is ok...

This is for generic boss mob:
Boot or DK, depending on flank > Twin > Demo > Frac > DK > True > Snap > Boot > Twin > BfB/IR > Demo > Frac > ToD > DK > Twin > Snap > HF
Repeat using BfB/IR whenever they are up, unless deliberately saving for LB.
Frac/Demo every second rotation, reapply ToD every third.
If more than one MNK present, use Boot instead of DK if debuff will last another rotation. The DK debuff is universal and does not stack.
Bonus tips:
If you know you will have to run away or the mob will teleport, apply a fresh Demo/Frac/ToD.
Auto-attack is a big honkin' deal, so stay locked on and facing the target as much as possible--unlock and cut through mobs only when necessary to survive. Remember: the rear/flank bonus applies as soon as you use a skill, not after the damage appears.
Alma, Are you throwing in ID as well and on fights where your stun isn't needed, are you using our non-charge stun for damage?
Can't really give you advice on rotation, you should of worked this out by the time your 50 really, I feel it shouldn't be something you analyze and pour over, should just know what to do by instinct ^^, people will debate till cows come home on their stuff, and what is better, I tell you just go with whatever you feel works, I just always make sure to keep twin snake, dragonkick TOD, Fracture, demolish up, 90% of time, sometimes they fall off for 1 or 2 seconds so its not end of the world, Also you can fit in 1-2 impulse drive EVERY combo rotation(if target is not running around everywhere (static)), Hope you work it out, MNK is fun and should be played from the gut
edit: just make sure your filling every GCD with at least something, and try to keep your GL on all time, dont sweat it when you lose it tho, happens often on primal hard modes!!!
I decided to stick with Blood for Blood and invigorate as my cross class skills for now.
...now to decide between impulse drive or fracture. I'm not good at this! :P
I currently use Mercy Stroke(nice for solo, extra off-GCD to spam for burst dmg), Blood for blood(use for LB, short cooldown, best cross class skill), Invigorate (run out of tp in most pro longed fights(gotta hit all those GCDs!)), Fracture(Better version of ToD, stacks nicely with ToD and demolish) and Impulse Drive (Can use 1-2 of these every combo rotation, either at the end before closing or at start before/after 1st combo attack, probably better to do them between twin snake and demolish or snap punchtho, so u can quickly refresh, requires more stationary fight to pull this offf perfectly, my ID's crit for 700 almost with Blood4blood up)
If you don't have some of these unlocked, the 20% def buff from marauder is good too, nice for popping when primals do their "2h" or for random unavoidable aoe's, every cd helps!.

Sorry for my ignorance (my monk is only level 35 atm) but I thought you could only choose 3 cross class abilities? I unlocked all of the ones you mentioned because I knew they were useful, but I wasn't sure how many I could pick at 50.
You get more as you level up. You get 5 at level 50.

how about gear, when look at darklight there is maiming and striking, Different is strike has skill speed + crit and the maming has skill speed + acc or altest the jewlery.
