PS3 Owners, How to request refund from Gamestop

For those of you wanting to request a refund. I spoke to someone from gamestop today and was allowed to return my preorder copy of FF 14 ARR. For those of you who want to wait it out and keep playing i sincerely hope things improve for you, but I would like to spend my money on something that is playable now. When things get fixed I may return. Here is the link i used. It helps to state exactly all of the issues you are having or know someone to be having with the game. (service accounts, 1017, 3102, etc etc.)

To return or not to return my old copy of the game cause I was forced to buy another one since it auto created me a new service account and SE would not respond to my tickets after a week.........

But I feel like that would be abusing it. But at the same time, it wasn't my fault o_o

side note: i'm sure this would work for PC hardcopies as well.
