Why not start with Bio since it's instant cast? Sure you have to wait on GCD but it's about the same duration as waiting for Bio II to cast initially.

Because then you're forced to use an off-GCD skill while your GCD is ready, which delays it, and it doesn't set your rotation up for the future.
Really it won't matter in a long fight what you start with, as long as all of your dots are kept in rotation after the first initial round, we are only talking maybe 20-30 points of damage (at my level anyways) based on initial casting. I think the most important thing is to keep an eye on your dot times and keep them up, more so than worrying about the rotation. All the dots are on the same GCD just like other classes. I know that by starting with bio > miasma > bio II > thunder > Ruin, bio just wears off and I am able to instant cast it again. (rotation at lv. 30).

I'm talking about Fester.
If you do Bio -> Miasma/Bio II -> Miasma/Bio II
You'll be doing fester after that Miasma/Bio II, and you will be on the GCD, and your next action is delayed. Using this rotation it will happen more often, which is what you DON'T want to happen, and the timers don't work as nicely as the rotation I posted.
Edit: Also, Bio does more DPS but less damage per cast. There are a lot of times this is necessary(along with the longer timer coming first being nice).
Ifrits jumping up to do his charges. Would you rather have Bio, or Bio II? You'd want Bio II, because after he lands you only need to apply Miasma/Bio before being able to use Fester again, whereas if you applied Bio then you'd need to re-apply all 3.

You'll learn to love Fester, and learning to properly utilize it and when you need to manage your mana(Energy Drain) is a big thing.
On a side note, I had a 1177 crit Fester yesterday. It felt nice.
