The GM is right here, though I would welcome a kick option.
Not every one can play nice, simple as that.
When you go with your FC/LS you can go with nice players, with a pug it is well random.
I had a good experience myself though.

Had a similar problem last night with Titan. Tank only spammed one move and didn't generate enough enmity. Healer was desperately trying to keep everyone alive. Tank blamed healer when he lost hate. But if the tank was using his other enmity abilities there would have been no issue. Tank continually insisted the healer was doing dps because the arcanist was casting DoTs (Bio). In the end the tank was fed up and left because of his own ineptitude. Tank probably subscribed to the thought that "Oh ARR is a simple spam fest so I'll just use my strongest ability for an instawin"
Unfortunately after he left, dungeon finder didn't prompt us to replace him. So had to leave the instance and search again. Luckily the next time around we had a better tank.

I'm going to say this, because i've been there, having pulled by accident and died, but the rest of the party sucked more than you. If any of them were above derp level, it would have just taken longer. One dps going down? Just slower damage. Tank or healer? that makes more sense. I've gone down in trials, near the end, and the rest of the party survived to finish. As long as you didn't pull several groups, the rest should be fine.
That said, not really a GM issue. Either leave and try your luck elsewhere, or don't leave and make him wait too.
It sucks, it's the healers curse sometimes. I did a run earlier, luckily we finished the dungeon, with a MRD tank, Summoner and Bard all from the same FC (according to them) the tank couldn't hold agro on the bosses, the bard didn't believe in CC (would just spam aoe and always seemed to wait until after i slept a mob) and neither the MRD or Summoner seemed to be able to move out of aoe. Ofcourse it's the healers fault when someone dies, even if they stood in the middle of the room during Garuda and got nailed by her one shot ability, it's the healers fault. I really don't understand the no kick option. I had a great group for Garuda on my second attempt. Great tank, good monk and a pretty good bard. We died because it still pretty much new to us and the bard got dc'd before our second attempt could start. Had no option but to disband the group. I get they don't want people to be mean and such but the carebear approach is just silly.

Disturbances that do not fall under the harassment category but are intended to prevent or hinder game play for others are considered inappropriate.
I would say what the Tank did fell under this. Though this would be subject to how the GM interprets it since their policies are not that well defined but definitely see this as going against the user agreement. GM, if was able at the time should have sent the offending tank and told him to play well with others or leave the group. FFXI GMs have been known to put people in the white room in FFXI for the most asinine reasons. Since this is new launch should make their presence known, jail a few people for doing dumb shit so people play nice.
