For me, I got:

HT (Legsweep+BFB)>ID (PS+IR)> Dis (X-pot)> CT (Jump)> Phlebo (DFD)> TT (SDD)> VT (Lifesurge)> TT (Poison pot)

Also, phlebo initial hit is affected by disembowel, you can also blood for blood and internal release your dots yes, but the buffs have to be active at the moment where you apply them.
