Problem is that if they restart everyone who is currently playing gets booted. These people all rush to relog. This places immense server strain, some servers probably end up having severe issues. Now it takes many hours longer than it would've for the World to re-open and run stable.
This is why they won't do it.
AFK timer, I have only seen 15 thousand people ask for it.
Anti-AFK macro. They're using one to log in, not hard to just leave it running and beat most AFK detection systems. Simply put, any fix is easily circumvented within minutes of implementation or risks breaking their servers.

Do like every other MMO developer does: put timeouts on inactive characters, if they stand in the same place and no actions are being performed for a long period of time then boot them from the server.
