Image Upload - An error has occurred.

Upon clicking the Add Image button on a blog post, a window pops up saying;
"An unexpected error has occurred."

If I attempt to upload even the tiniest 1x1 pixel jpg image, it'll give an error message of;
"An error has occurred."
and the image will not be uploaded.

Afterwards I've decided to just skip the upload, and just post the blog, but it gives me the error message "Fail!"
Then, I've realized for some reason it logged me off my lodestone page while I was trying to post a blog.
