I know of a lot of people who are having the exact same issue who happen to have the same ISP, I tried calling the ISP but the one who took the call only guided me into stuff that might have been the problem and couldn't find anything.

ISP: Verizon FiOS 50/25
Location: Dallas, TX USA
When: Consistently since 2.1 Patch
Frequent In-game lag spikes
ISP: Verizon FiOS
Location: Tampa, FL
When: Always, since 26 December, and gets worse during peak hours (~3pm to midnight PST). Before Christmas, I had no lag whatsoever; afterwards, it is almost constant.
Every 30 seconds or so, in-game lag spikes causing the screen to freeze for 1-3 seconds and then fast-forward to catch up. AoE/boss effect indicators not showing up on screen at all before the effect hits (this is especially amusing/infuriating with Twintania's Fireballs and Conflagrations, and Titan Extreme's everything). End-game content mostly unplayable.
ISP: Verizon Fios 50/25
Location: Riverside, California USA
When: Every 30 seconds or so, in-game lag spikes causing the screen to freeze for 1-3 seconds and then fast-forward to catch up. AoE/boss effect indicators showing up too late for me to react on screen before the effect hits. This just started about a week or so ago. End-game content mostly unplayable.
ISP: Cox Communications
Location: San Diego, CA USA
When: Occurs during crafting, gathering, doing end game events(Coil, Ultima EX, Primals EX, or crowded areas)
This issue has started AFTER the Patch 2.1 I did not have this issue previously and my husband and I and our roommate all play FFXIV together at the same time in our house and prior to 2.1 did NOT have this issue. I have spoken to them I upgraded my modem to docsis 3 dual band and upgraded my internet service as well with hopes to solve this but the problem still persists, which leads me to believe it is not an ISP issue here is my speed test. http://www.speedtest.net/my-result/3208852657
