When does the problem occur (always, only during specific times?): usually around 4-8pm ET but can start as early as 2pm and end at 10pm.

Instance dungeons are the worst, but I still get it out in the world.

I feel your pain. Even though I DC/lag all the time in game it seems worse in dungeons and I have lvled as a healer. I've had this problem since launch (when I could get on) but it seems to be getting worse. I lag/dc during off peak hours now too. I've followed all the advice I can find and nothing seems to help.
