Occurrence: Can occur anytime. Lag spikes are more frequent on weekends and evenings than during daytime during the week. Exceptionally bad on sunday 9/8 and sat-sun 9/24-25. Lag spikes occur anywhere from a fraction of a second to 25 seconds during an Ifrit(Hard) battle. Sometimes enemies do not appear and are not target-able for long periods of time, for example: Garuda(Hard), Behemoth.
Had a time warner tech visit with confirm # 101046292 and found no issues.
Note: My wife and I are both playing from this connection. We do NOT lag at the same time. It appears to be a server congestion issue.
Note 2: On one day, I believe 9/6, myself, my wife, and a friend on the same ISP several miles away all received error 90000 and were disconnected, while LS mates remained connected to the server.

This issue went away for 3 days after the major patch. It then came back. Unplayable. Connection is still unstable at all time of the day. Now I am getting more 90000 error than before.

- When does the problem occur?: Seemingly random times. Very sporadic. Occurs constantly throughout the time I'm playing. There's long pauses during combat, when speaking to NPCs, performing actions, mounting & dismounting my Company Chocobo, list goes on. I haven't noticed anything specific that causes these pauses. I thought that the number of mobs in an area was partly the culprit, but there are times that, even in incredibly crowded areas, things still run smoothly. On the other hand, during instanced quests, where there are typically very few mobs/objects in the area, the problem arises.
This is an issue I practically never saw in Beta Phase 3, only occasionally saw in Beta Phase 4, and now see all the time during release.
Also, I'm playing on PS3 (have been since Beta Phase 3), if that matters any.

Occurrence: Dungeons specifically, when running around the world people will stop in place then jet forward. I dont get disconnected usually, but I'll go from 500 sending to 0 constantly in both of the aforementioned cases.

This is an issue I never had in Beta Phase 3, but constantly got it in Phase 4 and launch being Error 90000'd every 2 minutes, lag spikes once every minute or so and predicting mobs next 10 moves leveling 2 jobs to 15 and one to 45.
EDIT: I am on PS3 if that makes a difference?
