1.0 CE Purchased in Past. Not showing up or receiving CE items.

I purchased 1.0's CE in the past. I played for a long while so I know my account registered the fact that it was a CE and not a standard edition.

Upon logging in to the Mog Station, I am presented with:

Other Received Bonus Items
・Onion Helm

When I log into the Square Enix Member's site:
Only my copy of FFVII I purchased from Steam shows up. When I try to add my 1.0 CE code, it is in the wrong format. XXXX-XXXX-XXXX-XXXX

I know it recognizes that I have the game because I am able to log in and play without having purchased ARR. I am receiving the Onion Helm in Moogle Mail.

I am not receiving my CE rewards as mentioned by the Lodestone post here

Players who own the FFXIV version 1.0 Collector’s Edition will receive the FFXIV: ARR Collector’s Edition in-game bonus items (Helm of Light, Baby Behemoth Minion, Behemoth Barding, and Coeurl Mount) for free. The in-game bonus items will be delivered via Moogle Letter when FFXIV: ARR launches.

So, the issue is that my 1.0 CE isn't being recognized as CE, but just standard
AND
my CE 1.0 rewards (Helm of Light, Baby Behemoth Minion, Behemoth Barding, and Coeurl Mount) are not arriving in my Moogle Mail.
