First off make sure that the game is in Gamepad mode - http://imgur.com/W93nw4S

Also you should of been prompted by the game to do a quick in-game calibration. I don't know off the top of my head how to force that prompt again but I can give you settings to paste in.

Please exit the game before you apply these.

Go to C:\Users\{USER}\Documents\My Games\FINAL FANTASY XIV - A Realm Reborn
Open FFXIV.cfg in a text editor

Under <SoundPlaySettings> (at the next free line below this)

If you notice that settings like above are already present and still dosn't work; you should attempt to uinstall the drivers and plug your gamepad into a different usb slot.
