What about if you ordered PS3 physical CE? My dad did and all he received in the moogle was the cait sith doll and mog hat, no CE items. Will there be a code for those items with the package of the game?

Turn it around. There's the code.
Go to your PSN and redeem it and you're good from there.
When you log in XIV they will tell you they will put in the thing and then it'll be in Moogle butt's mail lol.
Also you should go ahead and reg your One-Time password.
