O.K, so I ended up going to bed earlier to wait this all out. I was not able to do what Timberwolf had said until just a few minutes ago. I am requoting this so others can see:
This has worked and I am now playing with my sweet Ahriman mount. It may take you several minutes to reach a represenative but, she/he will be more than helpful to you. I had a wonderful lady add my Crysta to my account. Just make sure to reference the FFXIV Crysta problem and then go on about how your prepaid cards are not working, it seemed the lady knew exactly what I was talking about. I am reposting this in high hopes that many of you do this. This process takes around 15 minutes to do and its a lot better than waiting for 48 hours for Square to release some news on it, only in the end, to not get a single detail about the problem.Got it! All right, here's the deal. It's a workaround, they still need to fix thier $#!t, but this will get you playing in the mean time:
1) Get a hold of an Ultimate Game Card (best found at Walmart or Gamestop) for, say, $20.
2) Log into Mog Station/SE Account Management and attempt to add crysta with it.
*(99% chance of it giving you an error and rejecting the card - this is just to make a paper trail to be followed)*
3) Contact the VISA operator @ (650) 432-3200, ask for PlaySpan (read: Ultimate Pay)
4) Tell the following rep you attempted but failed at adding Crysta. They will ask you for the PIN of the card and for the email that your game is tied to. When they look it up, they will see your prior attempt - makes it fast and easy for them.
5) Confirm that you want your game card placed onto your FFXIV account.
*(Within 10 minutes, you'll have an email letting you know you've had Crysta added.)*
6) Log into your Square Enix Account Management page, click on "Crysta Passbook", and you'll see the 2,000 crysta added.
7) Continue to Mog Station, and go through the payment process again, this time using the Crysta that is now on your account.
As I said, this is a pain in the butt and roundabout way of doing it (Hopefully you're lucky and can get a digital card from Walmart.com or Gamestop.com), but it will (and has in my case) gotten me back online and playing. In the mean time... SE, get off yer @$$es and fix your payment system!

Been locked out for 2 days. I'm Canadian Credit Card with NA account, but living in Shanghai currently so i guess that why i was blocked. I had to find an online retailer who would sell me a digital ultimate gamer card. (Thanks OneCard.net!).
Then i would enter the code, in FFXIV system, it would even show as $20 USD click confirm and STILL rejected (i guess cause my account was blocked from so many attempts). So I just called the PlaySpan number above from one of the steps.. waited 5 mins and the rep manually flushed my card balance into my FFXIV account.
He also said "Oh yeah, we've had a huge call volume increase since yesterday, FFXIV is having tons of issues with this".
Anyway, I'm back in and hope this helps someone else out.
I'm thinking I'm just going to have to try this. I don't even have any special circumstances around mine getting rejected. I'm in the NA on a NA account using cards issued in the NA. So there wasn't really any reason for my stuff to get blocked, but it was anyways. I actually had just got done buying something, i got the email saying it didn't work, so i tried on square and it failed. And I know the card wasn't blocked or anything on my end because i then used that same card to pay for lunch.Quoted for truth.

Update: To Whom it May Concern...
I waited 24 hours. I called SE a second time before attempting to make a payment with a credit/debit card originating from the same country as my IP. The customer service representative told me to call my bank and make sure it wasn't blocking the transaction. I called my bank and they told me SE had billed me 1 dollar transaction fees for each attempt. So, I log in and try again with my american debit card and I get Error 401, yet again. I decided to attempt post 322 method but never got past the error message. This was the last straw, I called up the SE phone support for the third time and got mad. They "escalated the issue to the billing department" told me I would have to wait until I got an email back from them.
So, I tried this:

OK so Crysta is the only way.
I had to go buy an Ultimate Gamer Card, got it from Offgamers
I went and tried to apply it to UltimatePay Point, but no go, cause if you try too many times through them it ALSO locks you out.
I called their Customer service number (877)-703-6339 (Which was a pain to find). They pushed through the application of the game card to my Ultimate play account AND that pushed through the Crysta points onto my account.
I updated my payment method and payed with Crysta points, and now I have access. 5 hours later, and SE didn't solve it, UltimatePay customer service did.
Way to get completely outclassed in Customer service AGAIN Square-Enix.
Went to Offgamers, bought a hundred dollar card (don't want to have to do this again in 6 months).
Funny thing is, I tried paying for the ultimate game card on offgamers with my american debit card and it got refused, however they accepted my french debit card!
10 minutes later, I have a game card serial key via offgamers account (key is not region locked because it is a virtual gamecard, works on all regions!!).
Tried to buy chrysta with the game card key via mogstation ultimate pay option for ultimategamecards, got refused.
Called up Ultimate Pay and they were able to transfer my UGC credits into chrysta in less than 5 minutes.
This whole process literally takes less than 30 minutes to do (granted I had to stay up til 3am for 9am EST finally to roll around)
Anyway, good luck to yuh! I cerainly don't trust SE to get back to me about this issue any time soon and I feel stupid for not having done this first thing because the people at UltimatePay fixed in five minutes a problem that has taken days for SE just to recognize.

Just spent my chrysta. 90 dollars of the hundred got me 6500 Chrysta; enough for both 180 days subscription and the digital collectors edition upgrade. Hope this helps!

Not sure that will work for me, because I can't even attempt to claim a card for Crysta. As soon as I choose how much Crysta I want, I just get error 11000 before I get payment options... :/
Tried to call Ultimate Pay and got through, he did not want to help me and told me to contact Square Enix.
So while trying to purchase the game cards I got declined by Walmart as well. Thinking ok? Maybe it is my card? I called my bank. They said that I have no holds/warnings on my account and then went over some additional troubleshooting with my and decided that they had no idea why the card was denied. Address, name and phone number all match.
I'm totally baffled by this.
