Not even remotely close to worst ever. A lot of you kids probably don't even remember Anarchy Online.
ARR, at least, has a functional client, with all the major issues currently being caused by server-side failings. Anarchy Online didn't even have that.
Hell, FFXIV's 1.0 release was worse than this in a lot of regards. Again, when the server isn't completely losing it, ARR works just fine and is a lot of fun. The same could not be said for 1.0.

Still haven't seen a worse one than Diablo 3. Absolutely terrible. Destroyed my enthusiasm for the game. And then I actually PLAYED the game, and that only served to cement my lack of enthusiasm. Massive disappointment.
I'd say "no" to the worse launch ever simply because I have *nearly* error free on the Japanese servers. Thus far the +'s are the population is rather nice, the world looks very good, quests are entertaining, tons of tongue in cheek jokes (aethernet anyone?), the game play is smooth, classes are fun and distinct, there was thought put into each region to accommodate power levelers. The -'s are can't skip the character transit to the starting city, can't skip the starting quests (I know my way around by the 2nd alt in a town, thanks), I couldn't play with my bud on Diabloooo all day, no costume slot for a customized look, and sometimes gear progression makes me go whut (the S&M level 10 lancer gear?).
Who knows, they might offer free character transfers once everything has launched.

Anarchy Online. Prove me wrong.
To those saying AO was the worst, +1. The whole one big server mess (that was eventually made into multiple servers) and just bugs up the chocolate highway. It became a much better game as time went by, but the launch was absolutely horrid.
Was going to bring up this one myself, diablo 3 was really bad, expecially considering it isn't a true MMO. Probably wasn't the worst ever, anarchy online sounds awful based on this thread, but it was the game that sprang to mind for me in recent memory.
Vanguard: Saga of Heroes still holds the Crown for worst launch ever, it also holds the crown for biggest letdown, crown for greatest potential...unrealized and the Grand Master Trophy for Taking the Best Ideas from every MMO ever made, putting it into one game, then completely failing at making it playable in the first 3 months.
Hopefully the new engine falls through soon I'm getting tired of waiting...not enough people playing to even make a party anymore (unless it's for powerleveling hecks ~_~)
Worst launch I've personally experienced was probably gw2 honestly, I've never been affected by launches...or at least for the most part but I couldn't even get on to play gw2 for several days when it first came out lol I think it took a good week for them to fix that stuff. I played aion at launch but I never had to wait for a queue, I played rift and never had any of those weird issues, etc. and in this game I was fortunate enough not to have any issues besides the server being full which was just recently.

Yeah, this is the real frustrating part; like I said, the client is well-tested and just fine. Every single problem the game has in NA right now is related to server issues. Every single one.
There's a bunch of games, past and present - D3, Anarchy, Neocron, Conan - which can't claim to have been good games beneath the server issues. In that regard, this isn't nearly as bad a launch as the game itself is perfectly workable so long as the servers themselves aren't horribly overloaded. It's just the frequency of being overloaded in prime time that's the real issue.
