Issues Accepting Quest Rewards

I cant be the only person this is happening to. After about 7-8 quests from level 1-12 after completing the requirements to complete a quest, I speak to the person who holds the rewards and the accept button is blacked out and cannot be selected. For example there is a quest in the central shroud where you toss stale ale on moist depressions to battle six slugs for about 3100 exp and some items. I beat all the slugs and go back to get the rewards and its impossible. Am I doing something wrong or is this the game?

I have this problem too on multiple quests that require items to complete. I have all the items to complete the quests but it will not allow me to hit the complete button. this is very frustrating. I can't even complete my main Archery mission "A Matter of Perspective" because of this.

Any quest where the rewards list says "Options," you have to select one of the items shown in order to complete the quest. You don't get everything. . .

To be fair, players are probably used to games in which you click accept and then have to select the optional reward...
This one keeps popping up all the time for some reason...
