The token or app can only be added to your SE account if you have an active FF14 or FF11 service account. They most likely don't consider FF14 EA as "active" for this purpose because the official launch isn't until Tuesday. I'm guessing this because the NDA restrictions from Beta 4 are still in force. I was only able to add my OTP app because I currently have an active FF11 account. I plan on cancelling that account, but not until after Tuesday, just in case.
