Some things are impossible to translate so they make up their own thing. But why not do that for everything? (That's what I think, it's probably cheaper to do that)
It doesn't really matter for NPC's that don't have much to do with the world as a whole, but I have seen plenty of things within the main story quests that, well, do kind of tick me off.
Plenty of those were dialogues perfectly translatable without losing their context or comedic value but were still changed to something else completely.

I know from experience how hard some things can be to translate, but sometimes it looks like they aren't even trying to remain close to the original context.

If I must give a concrete example it will be in the Preatorium Gaius battle where the Japanese voice actor taunts you and the English dialogue sais "Aaaaaaaaaaargh!".
I mean, did they even try?

If I remember correctly, then the German version of X is a good example. The German subtitles are sooo different from the voice acting it's really hilarious xD" I heard that the German subtitles more follow the Japanese dialogues/version and this is why it's a bit off.

Nevertheless, I wished "Hast du jemals von Thal Icebound gehört?!" was in the German version Maybe I'll make a thread for it.
I went to Japanese because I just couldn't stand the "trying to be Brittish" accent of some of the VA's anymore, it was just too horrid to hear.
It's like listening to someone from the southeast of my own country trying to mimic my own Amsterdam accent.

As for that NPC dialogue, I think the German version actually might be a lot closer to the original Japanese text.
I'd like to see what the JP client shows. (And hope it's in Kana, I can read that.)

As for the Lipsync, none of the languages are, truth be told, they just make the lips move whenever the NPCs speak making the game look like you're watching an old English dubbed Kung-Fu movie.
