It's on a flyer that came from the XIII's box, behind the manual. It says "SQUARE ENIX MEMBERS" and "Become an exclusive Square Enix® Member by registering your copy of FINAL FANTASY® XIII". I don't see much else

That shouldn't be possible since all member codes are on that format... And if you already applied for ps3 version back in 1.0 beta then you don't need to worry about the application... I personally am not applying since I already applied for ps3 at 1.0...

Hi Wilan,
You need to use your flyer code to register your copy of the game first following the instructions on the flyer then you will receive an email with the promo code...
Goodluck!

I got an email from Square Enix Members after i originally registered XIII. the email contained the code for 2 purposes

I did not see anywhere on the beta application asking for Legacy status. I also wasn't sure if I should use the e-mail I have on my Square-Enix account (wasn't able to change it) or if I should use the e-mail I check most frequently. Was this beta bonus for being Legacy a lie?
Edit: checked the details section. It says Legacy members don't need to apply. I did apply, though, and I hope that doesn't cause any confusion...
You mean this note on the left side of the screen when you first go to the page?:I did not see anywhere on the beta application asking for Legacy status. "Players who met the Legacy campaign requirements can participate in the beta test starting from phase 1 and are not required to apply. However, we ask players who wish to participate in the PlayStation 3 beta test to apply for it."
Lemme guess, you didn't bother reading and just filled out the form?

I think you get given a different member code if you register Final Fantasy XIII on the Square Enix site using the one on the flyer. I wrote down a separate member code underneath. Also, yes! I didn't get into Alpha, so knowing I'm a Legacy Member, plus I have the XIII code, is very good news indeed!
Yrusama, I think you need to be logged in in order to apply, so they'll know which account you've used to register. The email you add is probably just for confirmation and contact.

The 4 four digit/letter code is a members code while the 5 four digit/letter is the code to unlock asura armbands and what we originally used to register for ps3 beta in 1.0...I think you get given a different member code if you register Final Fantasy XIII on the Square Enix site using the one on the flyer. If I recall the email was for the 5 four digit/letter code to unlock asura armbands and register for ps3 beta in 1.0
