Hello all!
Believe it or not, all three 'missing' character types have legitimate background stories as to why they weren't around in 1.0. These stories were actually conceived way back before the original launch, with plans on releasing all the juicy details via quests. Unfortunately a little thing called "Meteor" threw a wrench (spanner, for you sophisticated types) in those plans, and here we are.
While the lorelords have deemed it still a bit too early to release that forbidden knowledge, you can rest assured knowing that the stories will all be revealed in ARR. I personally enjoy the background for the female Highlanders. Let's just say they haven't been merely tending cookfires and mending tents these past few years. Those ladies are TOUGH.
As for naming conventions, you won't have to wait for ARR's release for those, as we hope to have a comprehensive guide out sometime during before or right after the launch of Beta. It will cover all 5 races (including both clans and both sexes) and should include a Roegadyn dictionary to help those looking to make a Sea Wolf.

tl;dr so forgive me if this was mentioned.
I think the Roegadyn were more-or-less a recent addition to Eorzea's populace. The Hellsguard may be from the northern mountains, but the Seawolf tend to roam the seas west and north of Eorzea (if I remember correctly). It always seemed like Eorzea (especially Limsa Lominsa) was a Seawolf's home away from home, and nearly all of the women were either left behind or simply stayed on the seas.
As for the male Miqo'te, I'm pretty sure Miqo'te in general were cloned with partial DNA from frogs, causing the all-female population to suddenly produce males. They then broke free from their captivity and terrorized a group of tourists, including Sam Niel, Jeff Golblum, and....
*man in a plain suit walks in and hands Yru a note*
...That's the plot to Jurassic Park? Whoops. Guess we'll just have to get a response from SE.
For highlander women, I think it's as simple as bodybuilding being more of a masculine thing to do. How many women in real life do you know that might fit the Highlander description? They exist, but they aren't all that plentiful.

We're supposed to pretend they've always been there!

The Highlander people are only from 1 place, Ala Mhigo. That's one explanation of why they're just now appearing. As for Female Roegydan, Hellsguard Roegydan are from the mountains to the north and Sea Wolfs are basically pirates roaming the seas. Typically that kind of travel becomes a male-only thing, sorry ladies. But ARR is a different game so women are rising up and doing their own things now, no longer being stuck raising the snotty nosed children.
As for male Miqo'te, a NPC in Ul'dah speaks of them and says they exist in Ul'dah but are rarely seen. Not to mention Seekers of the Sun are only out during the day and live in the Sagoli desert, while Keepers of the Moon are only out at night and live deep within the Twelveswood in Gridania.
But I'm sure Ferne has a more in-depth illustration of the how-and-whys.
