Details & Steps:
It begins on 28th September 2012. For a week, occasionally I faced R0 in-game. After a week, R0 becomes very frequent. R0 is very obvious during JP peak hours, especially when joining raids and crafting.
At first, I called my ISP regarding this R0 issue. Technicians were sent twice to my home. They checked on the condition and quality of my internet cable, and changed port. Nothing is solved.
By the third time technician was sent to my home, I am advised to install a freeware to trace and monitor my ping. Therefore, I begin my ping to 202.67.51.114. My results are as follows:

From screenshot above, it is known that ping is lost frequently. In order to detect where actually goes wrong, further test is done and the result is as follows:

From the screenshot above, packet losses occur at IP 219.117.X.X.
By searching at http://tools.whois.net/whoisbyip/, it is found that something is happening at Square Enix's network?

I have port-forwarded and enabled UPnP at router.
Ports enabled are:
TCP 54992 to 55551
UDP 443, 55296 to 55551
(Ports are re-confirmed open by using a freeware.)
I notice R0 starts at the end of the official subscription dateline.
Servers have no maintenance for quite a while now, too.
