How to die even if youre still dead:

1. Go at 18:00 at Camp Bluedust, kill there Atomos and loot the Stone until you get heavy. Make sure not to get speed run.

2. Port to Camp Brittlebark and hope that Atomos didnt spawn at the camp. Run with your Chocobo to Atomos spawn SW and pass the enemies there. Get hit and demount by one of the enemies. Try to run away and find out that you are still picked with heavy and youre too slow to run away. Die.

3. Make youreself a retourn in the hope to come back to the camp to get to atomos. Let your charakter stand up to wait to get retourned to your homepoint. Find out that you are in a teleport lag and standing there with weak and full hp. Get hit by the enemy that just killed you to let him the fun kill you again.

I remember way back this would happen. Die, do a return, find that the mob still responds to me and kills me again. You'd appear in a flash of light at the camp, sprawled on the ground.

^ this happened to me once and it wouldn't even let me accept raises because the game was so confused.
