Unexpected Errors trying to load FFXIV

I've looked at several others having the same issues as i am and no solution has worked or been capable of working.
It only effects me when im in large settlements (ie limsa) and when i try to play in dungeons/trials/content
Theres been very rare cases where ive managed to get into the game and im able to go to places like my ingame residence but outside of that i cant really do anything in the game, and i just barely managed to avoid losing my house after 2 months of not really being able to play and crashing everytime i try to go anywhere/do anything

The specific error i get is "An unexpected error has occured, exiting final fantasy xiv" Along with nvwgf2umx.dll+ and a random chain of numbers. ive got several screenshots of this happening if it might help. Is it a graphics card issue? outside of the crashes the game acts relatively normally, Ive tried using the repair in the launcher and reinstalling several times, reinstalling all my drivers, and im at my wits end with this.

In order get a better sense of the problem could you please provide the following information?

Could you provide the exact error message received?
Are you running any additional programs alongside Final Fantasy XIV?
Does this crash occur on more than one character?
When the crash happens, is the PC over heating at all?
Were any update or changes made to the PC at the time of the crash?
Does this crash occur if the game is run in full screen mode? Does it happen in borderless or windowed mode?

Could you provide a system report? Simply go to the 'Config' option on the Final Fantasy XIV launcher and to the right of 'System Information' click on 'Display.
