She was an existing character in the way Emet or Ysayle was an existing character. They had a big backstory with connections to the main plot and characters, but didn't actually exist in the game until they were needed so they could be a sacrificial lamb the players actually like to drive the story forward while tugging on our heart strings because the team doesn't have the guts to take out an actual scion.
Really don't agree that they took risks in ARR or HW. ARR was highly criticized by legacy players for them turning the game into a WoW clone. HW followed basically the same paint-by-numbers that ARR's endgame was.

I cant even speak for raiders anymore even though I see some regular PFs for it and what their opinion is about quantum, but for all the casuals I know nobody really gives a crap about it because it doesn't concern them anyway. The actual success is putting floor 99 of a deep dungeon closer to their reach though, i'll grant them that.

I find the raids and battle content fun still. But yes, they do need to take more risks with the game design and replayability of content in general, as well as the reward structures.
I'm just doing three tribal quests a day, and low level hunts for aetherite tickets at the moment. I'm seriously considering putting this game aside until the next expansion and going back to catch up in WoW after 3 years away.
Asking any company to take risks is a hard sell, but asking a JP company to break from tradition is like asking a cat to fly. "It's just how we've always done it" is like their corporate mantra over at Square Enix when it comes to XIV, and the game keeps making money no matter how poorly they mismanage it, so there's 0 financial incentive to change or even read the forum posts of baka gaijin like us. Hell, didn't they fire a bunch of their Western staff recently? I wonder if there's even anyone around who can read our feedback, lmao.
FWIW, I agree with you OP. This game needs a fresh take on the old approaches. But I'm just doomerpilled and don't expect it to happen. And even if they began listening to player feedback, I mean, jesus christ dude, have you seen some of the gameplay suggestions posted here? I'm actually scared of what'll happen if they open a thread at random and take feedback to heart, because there're some seriously dumb people here who should never design anything, asking for stuff that would ruin the game even more. Part of me doesn't blame the devs for just quarantining this place and never looking in.

I really agreed with this alot... stop making us do 50 steps every tier to get the armor online... Just make one set of armor.. you can have it change appearance as you upgrade it.. This has a lot of benefits because people have to start from the beginning to participate.. maybe they can upgrade more quickly as time goes on, reflecting how long the armor has been out.
Outside of taking risks and making the battles more exciting, I think things like 24 mans and 8 man raids would be a lot better if instead of new gear each time, we had a system were we upgraded the gear from the first tier and they added something other than basic stats. Gear in this game is some of the most boring gear in the mmo genre as a whole, they need to take a good hard look at how to make it better. It would also be a major boon if they were actual raids instead of just a few bosses with a hallway between them. If they would implement something like Dynamis or Assault from FFXI, with wide open zones, bosses that need to be popped by doing something and trash mobs that were actually worth fighting, it would be so much more interesting.
So tired of removing materia..crafting, trading in gear and re-melding for like 10+ classes and then crafters go through the same process just at different times. The whole system is stale and needs a complete rethink.
