Consistent FPS Drops based on Refresh Rate

I have a high-end pc and a monitor that is set to a 144hz refresh rate, however when i turn the frame cap on to "Main Display refresh Rate" i get massive frame drops from 144 fps down to sometimes 30 fps.
The drops occur consistently every 3 seconds or so and after about 1 second the fps go back up to 144.

As far as i know i already turned off all overlays, the refresh rate on the monitor is set to 144 and there are almost no background tasks running.

I think the cpu jumps a little aswell when this happens according to task manager but rarely goes over 35percent.

Pls help, i dont know what else to try. Thank you.

What GPU do you have? I would refrain from using "Main Display Refresh Rate", instead use "None" and then, open your GPU settings app (Nvidia App) and locate the driver setting for "Maximum Framerate" and set it to just below 144, for example 142.

Doing so will prevent G-Sync causing issues when the FPS go over 144, etc.

Make sure to turn off V-Sync in the driver setting as well.

Use Borderless Window mode only when FPS is set to "None" and your GPU can handle 80+ minimum FPS, 60 or less usually creates stutter and other issues.
I would use Fullscreen mode if you want to cap FPS at 60 on a high refresh rate monitor (120Hz+), also disable V-Sync manually as above.

"None" option disables V-Sync in the game only, driver setting overrides the game setting, so make sure to check that too.

Edit: Actually now if i put the fps ingame to 60 there are drops now aswell down to 20fps.
    Thank you for the reply!
I tried what you said. The fps is now set to no limit but limited to 142 by nvidia and V-Sync is off but the issue persists.

My graphics card is a Nvidia Geforce RTX 5070ti

Actually now if i put the fps limit ingame back to 60 i now get fps drops aswell down to sometimes 18fps.

In the Windows OS, navigate to: Start > Settings > System > Display > Graphics.

In this menu, use "Add desktop app" then locate the game folder, in the folder you'll see two sub-folders, select "game" and then click on the "ffxiv_dx11.exe" and add it to the list.
Once added, select it in the list of programs/apps "Final Fantasy XIV", set it to use "Nvidia Geforce RTX 5070 Ti" and flip the little button below to On or Off for the Optimize Windowed games, I would try it set to On and test the game, then Off, you may get some results, it's hit or miss, worth testing.

Secondly, if the above doesn't yield results, you can try setting the game to Fullscreen mode in the in-game Display setting menu, this should fix the issue of random framerate fluctuation, but is not ideal, try it and see if it improves.

I followed the instructions. Issue still persists.
I find it so strange that when i lock the game to 60 fps those stay consistent but when it goes higher the frames drop into the abyss.

Ok, i belive the issue has been found: Its not just a ffxiv-thing. Apparently the cooling of my cpu is broken causing my pc to get trottled as to not overheat which is creating performance issues in EVERY Game.
A Technician has been called to look into the hardware.

    Thank you for your help!
