Double Materia Meld Glitch

I got a friend to attempt a double meld on my Giantsgalls Cane and there was no animation for the success or fail where it shatters however on my friends screen and mine he raised his hands in the air like what happens when succesful and said succesful on his screen but now it isn't in his inventory after the game shows that it was a success with the hands in the air. Also after this he was unable to move have I lost my cane and if I have that is an unfair glitch losing the weapon after the last animation shows a success. Personally now I want a replacement Giantsgall Cane with double Vitality 4's as that was what was being attempted at the time

Steps:
1.Traded Giantsgall Cane and Vitality Materia IV plus catalyst to Dzian Cielo so would have +34 Vitality on the cane
2. Dzian Cielo attempted the double meld
3. No animation came up with the white sphere, however he raised his hands in the air the same way as when a successful meld is made and also said successful on his screen.
4. He checked inventory the cane had disappeared and also his character was then stuck
5. He called a GM who said couldnt help with the problem but to fill out a bug report and to log back in and out
6. Dzian Cielo logged out and back in this fixed the stuck issue however still no Giantsgall Cane in the inventory list
