Supply and demand. Even for its rarity it's an ugly helm so there is a surplus of stock compared to those who actually want it.
Below we have a transcription of what Naoki Yoshi-P Yoshida said at PAX:
- "For some players, like me, I kind of get sleepy because it's so repetitive."

it's not that, its because it's only useable by tanks and only from level 50 (the Peregrine Helm along with the proper Onion Helm are level 1 and equippable by all classes). But unless you happened to be an old 1.0 player who had the collector's edition of 1.0 and used the item code on your account (or tracked down an unused code) and thus have the proper Onion Helm, or were one of those very lucky 1.0 players that managed to source a Peregrine Helm in 1.0, the Red Onion Helm is the only way you can do a proper FFIII Onion Knight glamour.
Of course though with the upcoming changes to the glamour system coming in patch 7.4 then that might mean all of a sudden it will start being in demand again.

No I think they're right, the dated ugly design has a lot to do with the low price. Otherwise we would have already seen a significant price spike when they announced the upcoming glam changes.it's not that, its because it's only useable by tanks and only from level 50 (the Peregrine Helm along with the proper Onion Helm are level 1 and equippable by all classes). Yeah, I checked my server. 220K gil. That's really nothing for the rarity. Though now I want to make an Onion Knight glam.
