Unless you had your Journal open, you might have never seen it. Most folks who are caught up on current patch content don't bother with the Journal, they just run off the MSQ indicator or the quest icons directly in the zone.

This actually was a problem all the way back in Heavensward once, now that I think about it, for an ARR quest in Mor Dhona, where a quest icon showed as "green" in the journal but would appear as red when you went to talk to the NPC, but back then it was because it was level locked on the NPC but didn't have the level check in the journal. Most players wouldn't run into it because you're well past level 50 by the time you hit 45-47 MSQ these days. I only noticed it on an alt that was doing pure MSQ with no side content.
