I'm not a GM or anything, but I would suggest:
Disabling windows automatic update (temporarily while you troubleshoot this if it makes you uneasy) To do so: Open Change device installation settings (just search through start menu), and select "No" and then click "Save Changes"
Downloading DDU (Display Driver Uninstaller) and running it.
Reinstall your drivers using whatever NVIDIA installer you usually use.
My reasoning for this is your dxdiag is crashing and not running all the checks it should. "DxDiag Previously: Crashed in Diagnostics (stage 1)."
For some godforsaken reason, Windows has gotten this nasty habit in the last year or two of trying to "fix" graphics drivers, which usually ends up corrupting them. Disabling the automatic update stops windows from trying to "fix" your drivers and corrupting them.
If the above steps don't work, you can open "Change device installation settings" and turn it back on, though I recommend leaving it off and manually updating things. Windows was updating my GPU while I was in game until I turned this setting off, crashing my games and making me have to sit around and clean reinstall my drivers randomly...
