1. Be playing a character that is either a Miqo'te, Au'ra, or Hrothgar.
2. Equip Vanguard Armor of Fending or Vanguard Armor of Maiming.
3. Observe location where tail should be.
4. Tail is missing.
(Further testing may be required to see if other armor sets from this dungeon also have issues like this. So far I've found that the Vanguard Jacket of Casting, which I also got as a drop in the dungeon, properly displays the tail. And someone else's ticket shows an issue with a hat on Miqo'te.)
