


I found the problem, but I would rather post this in shame being wrong than not helping anyone and staying quiet.
At some point, and god knows why, in the NVIDIA control panel I had a NEGATIVE LOD BIAS set to CLAMP. I've had this game installed for years on my PC without an issue and this was the only setting marked differently for this game specifically. No doubt at some point I followed a guide and changing this setting solved something. What it solved I can no longer tell you, but it stands to reason that people experiencing this issue may have done the same, and have just forgotten this setting solved something in the past.
Change the NEGATIVE LOD BIAS from CLAMP to ALLOW (Global Setting) and restart the game. I am sorry I was wrong. And It was related to LOD, just not in game, it was in the Control Panel for NVIDIA.
This is how the character looks now, while not crystal clear on the hair its livable now.

No mention of GPU drivers anywhere. Update your GPU drivers in addition to changing clamp to allow.
