Eye patch(s) seem within the realm of doable. They're basically just "glasses" with one eye, same with monocles. However most of these are on the "hat/helmet", so to add such a thing, requires setting a priority with the hat object (eg does this replace geometry on the hat.)

Piercings are unlikely to ever be a thing, because that is pretty much asking for an additional face type. Tattoos are done by just adding an additional texture layer to the skin, but a piercing actually requires an attachment point (see the earring, and where it attaches on the different races.)

Pretty much the game wasn't designed to have piercings, and given how tumultuous players are about wanting to keep the same face, it's very likely that it could only ever be done if they break the individual parts on the model (Eg lips, ears/horns, nose) to be separate. Otherwise the "piercing" is just going to look flat. Look at the beast tribes and how seemingly things made of metal are bending over their geometry. Look at player characters how necklace objects supposedly made of metal are bending like they're fabric. That is what would happen.

The game doesn't do a particularly good job at not warping metal gear, in the first place, so I could only imagine any attempt to add small details that aren't skin-agnostic (basically things that are like tattoos/beauty marks) resulting in people not liking the combination of things and the developers being helpless to figure out what the players actually wanted.
