We don't know. It's especially difficult to determine on the PS4 version due to the lack of in-depth graphical settings with which to try and find some manner of combination that counters the issue.
All I can personally say is that I had no problems like this during my 6+ years of playing FFXIV on the PS4.
Same here, everyone is different, I am sure there are some people that had similar issues with the old graphics that are now potentially fixed for them.
I think its an individual kind of issue, which is why they need to add options (whatever they maybe) to help those that are having "eye strain".
My problem is why is the text for stuff really thin and small? the player, minion, location, npc names are all thin and small, its causing me such struggles trying to read anything, even in the chat area the "General, battle,event" tabs are small.
TSCMAA adds motion blur when the character is moving, it is VERY slight tho so maybe your eyes just have a skill issue? Also DLSS and FSR do NOT do anything as long as you have Enable dynamic resolution UNCHECKED.

My eyes are usually pretty sensitive when it comes to eye strain from games with bad graphical settings, I didn't really feel anything extra strain wise from my session playing the past few hours, however i also was up/down a bit sorting out food and chilling with pets so wasn't entirely playing the whole ~4hrs or so.
So it might be a setting of some kind you've got on? Usually good to check the usual suspects that cause the headache inducing strain to better tailor your experience to not be a painful one.
Remember to turn on night-light or something to turn down the blue-light on your monitors.
If your eyes burn after prolonged screen usage the blue-light is often to blame.
That's what was so confusing to me, I play with full lights on and my glasses have a blue light filter that normally helps a lot with this sort of eye strain.
I actually have my glasses coated to filter out a lot of the blue light. Yes it cost more than I was happy about but it helped so much.

Unrelated: I have already told my sister not to resubscribe until we know what is going on with this and the flying tilt gets either a tweak or on off switch. She had a very serious head injury 20 years ago. She is highly susceptible to motion sickness, eyestrain and migraines. I can tell by how it affects me (and I don't generally have issues) that for her she would be sick in minutes.
