If you mean 'you go to 1 HP and have to reach full HP to 'succeed' the mechanic', Seat of Sacrifice has one? But you have like 25 total seconds to get to full HP, and it happens exactly once (at the start of the fight when you have every CD available) so it doesn't really count, does it

I haven't even unlocked that one yet, but that's exactly why I said "actual". Because I vaguely remember seeing one in Dawntrail before, but it did laughable damage.

You had that in P1N and S too, and I'm pretty sure others had that since but I'd need to check a list.

Also for the Almagest type raid damage, you had Silkie's Total Wash which was basically Almagest with a cute skin.
